PPO Plan Participants: Get Guidance When You Need It with Guardian Nurses

Considering surgery or facing a complex health decision? You don’t have to navigate it alone. The Trust’s Guardian Nurses program connects eligible PPO Plan participants and their covered dependents with experienced Mobile Care Coordinators (MCCs) who can provide one-on-one guidance through your diagnoses, surgeries, hospitalizations, and more.

Your MCC nurse can help you understand your options, prepare for conversations with your healthcare providers, and navigate the healthcare system—giving you the information and support you need to make informed decisions about your care.

Guardian Nurse services are voluntary, confidential, and fully paid for by the Trust.
