To help participants who have been diagnosed with chronic conditions manage their health, the Trust offers Condition Management for eligible employees and their covered spouses, 18 years of age and older, who have been diagnosed with:
- diabetes,
- heart disease,
- asthma and
- other pulmonary diseases.
Selected participants will be invited to work one-on-one with a condition management nurse through the Guardian Nurses Mobile Care Coordinator (MCC) program to improve both their health and their quality of life.
From practical changes you can make to improve your health, to greater confidence managing your chronic medical condition, the plan you design with your MCC nurse will address your needs as a whole person, not just your disease or condition.
How it Works
If you qualify, your MCC Nurse will call to invite you to engage in this program. Your nurse will conduct a brief health condition assessment over the phone, then set up a regular schedule of calls or in-person visits with you. Your nurse can also go to your healthcare visits with you to participate in discussions between you and your care team to make sure your goals and plan of care is understood and followed. Your MCC nurse will be with you every step of the way.
If you are HRA eligible and are working with your MCC nurse for a diagnosed chronic condition, you can receive $30 a month into your HRA account, up to a total of $270. In addition, eligible participants who are enrolled in Condition Management and actively engaged in the program, can earn $300 in annual HRA funding.
The Condition Management program is intended as a supplement to, not a substitute for, your healthcare provider’s care. For diagnosis and treatment, always consult your healthcare provider (using doctor or physician omits nurse practitioners).
Condition Management is completely confidential and provided by Guardian Nurses Mobile Care Coordinators (MCC), an independent health management service provider.
