To: Sound Health & Wellness Trust PPO Plan Participants

The preferred provider network for your plan is changing as of January 1, 2026 from Aetna Choice POS II to Premera’s BlueCard PPO network.

What is Changing

The preferred provider network that you use for your plan is changing. This is the only change to your plan. Effective with any medical care services you receive starting on January 1, 2026, your preferred provider network will change from the current network, Aetna Choice POS II, to Premera’s BlueCard PPO network. All services you receive January 1, 2026 and after will need to be provided by a Premera contracted provider, so you will want to verify that your provider is part of the Premera BlueCard PPO network in order for you to receive in-network benefits.

How Will You Know if Your Provider is in the Premera Network?

You can contact your provider and ask them if they are in the Premera BlueCard PPO network or call the Trust Office claims customer service representatives at (800) 225-7620, option 2 then option 2. You can also visit https://www.premera.com/shared-admin and click on “Find a Doctor”.  The network is preselected to BlueCard PPO.

This link will also be on the Trust’s website after January 1, 2026.  Once the link is updated, you can search for Premera providers by going to www.soundhealthwellness.com, clicking on “Browse the site for PPO”, then clicking on “Find a Provider” under the Benefits section.

The Trust is Not Moving to Premera Insurance

The Trust is not moving to Premera insurance, but only using Premera’s preferred providers through their BlueCard PPO network. The Trust Office at Zenith American Solutions will continue to handle all of your medical claims processing, including your HRA (if eligible), and will provide all telephone customer service to you just as they do now.

There is No Change in Your Benefits

Your medical benefits are not changing with this new preferred provider network. Your benefits remain the same.

New Trust Health Plan Identification Cards are Coming

You will be receiving  new Trust plan identification cards, which will reflect the change from Aetna to Premera. When you receive your new cards, please throw away your old cards and make sure to show your new card to your medical care providers when you see them.
