Develop a Personal Plan for Healthy Living
Your annual Health Profile is a quick, simple, confidential questionnaire that gives you an immediate snapshot of your current health and health risks, and gives you a personal plan for healthy living.
Unlock your HRA Funding
Each year, eligible Sound Health & Wellness Trust participants and covered spouses 18 years of age and older have the opportunity to take their Health Profile to unlock their Health Reimbursement Arrangement funding.
Taking the Health Profile is the first step to help you avoid a higher deductible and to learn more about your health status. New in 2026 – You must take the Health Profile to earn annual HRA funding! This means that the health related actions you take throughout the year will only count toward funding your HRA if you complete your annual Health Profile between January 1, 2026 – December 31, 2026.
To make it easier to complete your Health Profile and get your full HRA funding, you can now take your Health Profile any time! Login to your MySoundHealth account to get started!
Quick and Confidential
The Health Profile begins with a quick questionnaire that you can fill out online, or by requesting a paper copy to be mailed to your home. Your responses to the questions are completely confidential, protected by federal law, and cannot be shared with the Trust, your union or your employer without your permission. Be sure you have your Kaiser Permanente ID card handy—you’ll need your member ID to take the Health Profile. If you have not yet registered for Kaiser Permanente Online Member Services, please follow the instructions on the website to sign up.
Useful Information, Instantly
The process takes about 15 minutes, and the moment it’s complete, the online system performs an instant analysis and presents you with your very own profile that not only rates your current health status, but projects your potential future health problems as well.
More Health Options
When you submit your Health Profile, you may also qualify for Sound Support services designed to deliver high-quality, personalized support for better health. These programs are offered at no additional cost to you, and are a great way to get extra support.
Remember, information on your health status and health conditions is shared only with Kaiser Permanente and the independent service providers that manage the Sound Support programs; it is not shared with the Trust, your union, or your employer. By law, this information cannot be used to determine or deny health care coverage.
