The Annual Health Assessment is a quick, simple, confidential questionnaire that gives you an immediate snapshot of your current health and health risks and gives you a personal plan for healthy living. Depending on your results, you may qualify for a variety of Sound Support programs.

In addition to learning about your health status, taking the Annual Health Assessment can help you avoid a higher deductible by unlocking the funding for your 2027 Health Reimbursement Arrangement (HRA) account.
Three Easy Steps
To complete your Annual Health Assessment, you will be asked to 1) answer a few questions about your health status, 2) select or confirm your primary care physician, and 3) confirm your contact information. The process takes about 15 minutes.
Any information you supply is completely confidential, protected by federal law, and cannot be shared with the Sound Health & Wellness Trust, the union or your employer without your permission.

PPO Plan Participants
The Personal Heath Assessment (PHA) is available all year long. Complete your PHA, select your primary care physician and confirm your contact information by December 31, 2026 to unlock of your 2027 HRA funding.
Kaiser Plan Participants
The Health Profile is available now. Complete your Health Profile, confirm your contact information (if the employee) and select your primary care physician with Kaiser Permanente by December 31, 2026 to unlock your 2027 HRA funding.
