In addition to learning about your health status, taking the Annual Health Assessment can help you avoid a higher deductible by earning a significant amount of funding for your 2024 Health Reimbursement Arrangement (HRA) account.
Three Easy Steps
To complete your Annual Health Assessment, you will be asked to 1) answer a few questions about your health status, 2) select or confirm your primary care physician, and 3) confirm your contact information. The process takes about 15 minutes.
Any information you supply is completely confidential, protected by federal law, and cannot be shared with the Sound Health & Wellness Trust, the union or your employer without your permission.

The Personal Heath Assessment (PHA) is available now. Complete your PHA, select your primary care physician and confirm your contact information from July 1 to Sept. 30, 2023 to earn part of your 2024 HRA funding.
Kaiser Plan Participants
The Health Profile is available now. Complete your Health Profile, confirm your contact information (if the employee) and select your primary care physician with Kaiser Permanente from July 1 to Sept. 30, 2023 to earn part of your 2024 HRA funding.
