Real Support for Chronic Health Conditions

To help participants who have been diagnosed with chronic conditions manage their health, the Trust offers LiveWell Condition Management for eligible employees and their covered spouses, 18 years of age and older, who have been diagnosed with diabetes, heart disease, asthma and other pulmonary diseases. Participants receive helpful information by mail and may also be invited to work one-on-one with a condition management nurse to improve both their health and their quality of life.

From practical changes you can make to improve your health, to greater confidence managing your chronic medical condition, the plan you design with your personal nurse advocate will address your needs as a whole person, not just your disease or condition.

How it Works

Participants will get 24/7 access to a nurse advocate, and some will receive scheduled outbound intervention calls as well as clinical device monitoring. In addition to these materials and services, the program provides a confidential and secure interactive website with personalized tools, an online library and other resources.

If you qualify, you will receive a call inviting you to participate in this program and a specialist will contact you to conduct a brief health condition assessment over the phone. You will also receive a program welcome packet by mail. Participants who are working with a nurse advocate for a diagnosed chronic condition can receive a check for $20 a month up to a total of $180 per year for participating.
