Real Support for Chronic Health Conditions

To help participants who have been diagnosed with chronic conditions manage their health, the Trust offers LiveWell Condition Management for eligible employees and their covered spouses, 18 years of age and older, who have been diagnosed with diabetes, heart disease, asthma and other pulmonary diseases. Participants receive helpful information by mail and may also be invited to work one-on-one with a condition management nurse to improve both their health and their quality of life.

From practical changes you can make to improve your health, to greater confidence managing your chronic medical condition, the plan you design with your condition management nurse will address your needs as a whole person, not just your disease or condition.

How it Works

Participants will get 24/7 access to a nurse, and some will receive scheduled outbound intervention calls as well as clinical device monitoring. In addition to these materials and services, the program provides a confidential and secure interactive website with personalized tools, an online library and other resources.

If you qualify, you will receive a call inviting you to participate in this program and a specialist will contact you to conduct a brief health condition assessment over the phone. You will also receive a program welcome packet by mail. If you are HRA eligible and are working with a nurse for a diagnosed chronic condition, you can receive $30 a month into your HRA account, up to a total of $270. In addition, eligible participants who are enrolled in Condition Management telephone nurse coaching and actively engaged in the program, can earn $300 in January 1, 2019 HRA funding.
